Your Child's First Appointment
At your child's first appointment please bring the completed patient setup form and patient privacy rights form with you. These forms can be found below.
Transferring from Another Practice
If your child is coming to us from another pediatrician we will also require their entire medical record prior to the appointment. Please complete the medical records request form, and return it to us as soon as possible.
Please allow ample time before your appointment to request your child's medical records. In our experiance medical records can take as much as 2-3 weeks to be processed by some clinics.
