New patients to Nadal Pediatrics are required to fill out the above listed forms. Once you have booked your child's appointment please take the time to fill out these forms prior to the day of the appointment. Also, if you need to request records from another doctor's office please fill out a Medical Records Release Form, and mail it to the doctors office you are requesting records from.

On the day of your child's first appointment please remember to bring a copy of your child's shot record and Insurance card.
